Rise Pathways is a comprehensive community initiative designed to support individuals transitioning back into society after incarceration. The program provides coordinated medical care, behavioral health services, addiction treatment, case management, and workforce training to help participants restore their health, rebuild stability, and create sustainable career opportunities.
